Admission Requirements for new student:
A student applicant must:
1. Be a graduate from a duly recognized secondary school
2. Be of good moral character as attested by the certification from the Principal / Guidance Counselor or Class Adviser
3. Satisfactorily passed the school’s entrance examination, consisting of the college’s assessment exam and interview
*Examination schedule is from Monday – Saturday, 9 am – 6 pm at the Admissions Office
4. Be physically and mentally fit
5. Must submit the following requirements:

Original copy of Form 137-A
Original copy of High School Report Card; Form 138
Certificate of Good Moral Character
Photocopy of Birth Certificate (NSO)
If married, photocopy of Marriage Certificate
1x1 picture (2 pieces; white background)
2x2 picture (2 pieces; white background)
Physical and Medical Examinations at St. Dominic Medical Center
Admission Requirements for Transferee or 2ND Course Taker:
Entrance Examination *Examination Schedule is from Monday to Saturday, 9 am – 6 pm at the Admissions Office
Official Transcript of Records or Copy of Grades
Certificate of Good Moral Character
Honorable Dismissal from your former college / university
Photocopy of Birth Certificate (NSO)
If married, photocopy of Marriage Certificate
1x1 picture (2 pieces; white background)
2x2 picture (2 pieces; white background)
Physical and Medical Examinations at St. Dominic Medical Center
Admission Requirements for Foreign Students:
A foreign student – applicant must:
1. Be a graduate from a duly recognized secondary school in the Philippines or abroad
2. Satisfactorily passed the entrance examination consisting of the college’s assessment exam and interview
*Examination schedule is from Monday – Saturday, 9 am – 6 pm at the Admissions Office
3. Be physically and mentally fit
4. Pay the Foreign Student Fee
Must submit the following requirements:
Photocopy of Birth Certificate
Alien Certificate of Registration (ACR)
Student Visa
1x1 picture (2 pieces; white background)
2x2 picture (2 pieces; white background)
Physical and Medical Examinations at St. Dominic Medical Center
Admission Requirements for Caregiver Program Students:
1. Must be a High School Graduate and of 18 years of age
2. Must submit the following requirements:
• Photocopy of Birth Certificate
• Photocopy of Report Card (if High School Graduate)
• Photocopy of Transcript of Records (if college level / graduate)
• Photocopy of Marriage Certificate (if married)
• 1x1 picture (2 pieces; white background)
• 2x2 picture (2 pieces; white background)
• Physical / Medical Examination at St. Dominic Medical Center
Admission Requirements for Medical Transcription Program Students:
1. Must be a High School Graduate and of 18 years of age
2. Must submit the following requirements:
• Photocopy of Birth Certificate
• Photocopy of Report Card (if High School Graduate)
• Photocopy of Transcript of Records (if college level / graduate)
• Photocopy of Marriage Certificate (if married)
• 1x1 picture (2 pieces; white background)
• 2x2 picture (2 pieces; white background)
• Physical / Medical Examination at St. Dominic Medical Center
